Adult Day Health Center
Surrounded by your entire care team, we offer a safe place to share socialization with friends, exercise and wellness, family style meals, and so much more.
After Hours Medical
On-Call Care
The Buckeye PACE clinic is open Monday-Friday 8a-5p for your clinical needs. For clinical needs after hours during the week and on the weekends, the clinic is just a phone call away
Transportation
Transportation to all medical appointments & to and from the Day Center is provided. We accommodate special transportation needs and will help you get ready or settled back at home before we leave.
Collaborating closely with each participant and their family members, our Interdisciplinary Team (IDT) takes a holistic approach to developing the most appropriate and personalized care plan to meet their unique emotional, clinical, and social needs. Learn more about our programs.

Eligibility and Cost
Buckeye PACE serves seniors from all walks of life who meet the following eligibility requirements:
55 years and older
Live in designated counties served
Meet state nursing facility level of care criteria
Live safely in the community with PACE services at the time of enrollment
Meet any additional program-specific eligibility conditions imposed under the PACE program agreement.

About PACE, Program of All-inclusive Care for the Elderly
The Program of All-Inclusive Care for the Elderly (PACE) provides seniors with an alternative to nursing facilities and enables them to live in their homes. PACE is also credited with improving the emotional well-being of participants.
With PACE, a team of health care professionals works with each individual to make sure needed services are coordinated. PACE programs get to know their participants and design services to meet each person’s unique needs.
