Disenrollment Information

The Buckeye PACE program is voluntary for the individuals that choose to enroll in Buckeye PACE. Because of this, a participant may voluntarily dis-enroll from Buckeye PACE by communicating this to the Buckeye PACE team with as much advance notice as possible. Even if shorter notice is provided a participant's voluntary disenrollment is effective on the first day of the month following the date the PACE organization receives the participant's notice of voluntary disenrollment.

Until a participant is dis-enrolled from Buckeye PACE, all services provided for the participant must continue to be authorized by Buckeye PACE as Buckeye PACE remains liable for payment. The Buckeye PACE team will coordinate all care planning activities to primary care and other community resources that the participant may be discharging to.
