Step 1: Intake and Initial Assessment
Once we believe that you meet the basic requirements to join Buckeye PACE, we’ll schedule an in-home visit. During this visit, we’ll walk you through the program in detail and determine if you may meet the medical criteria for enrollment. We will also assess if you would qualify financially in order to receive PACE with no out-of-pocket costs or copays.
Step 2: Assessment and Application
If you appear eligible and you’d like to explore if PACE is an option for you, our intake team will begin gathering your medical records and history from your current providers. From there, you’ll receive a comprehensive medical assessment by Buckeye PACE to confirm your medical eligibility. If this assessment determines that you do not qualify for PACE, you will be notified and have the right to appeal the decision.
Step 3: Enrollment
If your application is approved, you’ll be eligible to enroll in Buckeye PACE beginning the first day of the following month.
Throughout the process, our team works closely with you and your loved ones to ensure all paperwork is completed. You’ll also meet your new care team and begin creating a personalized care plan tailored to your specific needs.
