The Buckeye PACE team of physicians, nurses, personal care assistants, activities staff, therapists, dietitians, social workers, and transportation workers provide the basis for how each enrollee’s “Individualized Care Plan” will be administered.

Your Buckeye PACE Approved Care Plan may include:

The Cost of the Buckeye PACE Program

Buckeye PACE offers healthcare, medication management, transportation, social activities, in-home support, and other specialty care services. All individuals aged 55 and older who live in our service area are eligible to join.

We can help you determine your eligibility for Medicare and Medicaid and assist with the application process. A private pay option is also available.

Participants in Buckeye PACE must receive all necessary healthcare—including primary and specialty physician services (excluding emergency care)—from providers contracted with Buckeye PACE. Participants may be responsible for costs of services not covered under the Enrollment Agreement or those received outside of PACE.
