At Buckeye PACE, we serve as a single-source provider of personalized and cohesive health care and social services, planning, and delivery to seniors who need help managing their day-to-day care so they can remain living in the comfort and safety of their homes.
Each Buckeye PACE Interdisciplinary Team (IDT) is comprised of multiple clinical and service professionals, including physicians, nurses, personal care assistants, activities staff, therapists, social workers, dieticians, transportation workers.
Together, the IDT staff ensures every level of care is tailored to each participant’s specific needs and properly provided.

About PACE
Program of All-inclusive Care for the Elderly
The Program of All-Inclusive Care for the Elderly (PACE) provides seniors with an alternative to nursing facilities and enables them to live in their homes. PACE also supports improving the emotional well-being of participants.
With PACE, a team of healthcare professionals works with each individual to make sure services are coordinated.

For your convenience, we coordinate your medical appointments, including scheduling, transportation, and post-appointment follow-up and support.
